Select a date and time for your appointment using the calendar below. / Step 2 (below) must be completed with payment to hold your appointment.
Part of this process includes creating your Patient Portal account. The instructions for setting up the Patient Portal can be found
here.
Please contact your airline to confirm the date and time you need to provide your test result to them. Then, schedule your test appointment accordingly.
Flights with multiple stops or layovers may require you provide test results to them before the first flight departs.
温馨提示：周五早上10点以后到周日取样，IgM结果出在下周一的可能性很高。PCR周六中午10点之后和周日取样，结果出在周一的可能性很高。
爱家诊所强烈建议大家周一到周五上午10点前取样。
航空公司的要求可能会和使馆不同，请大家一定和航空公司确认好，再安排行程和取样时间！预约前请和航空公司确认航空公司的要求。航空公司可能会和使馆有不同的要求！不同航空公司也可能有不同的要求！

Your appointment will not be confirmed until you complete the form and make your payment in Step 2.
