第一步：预约并交费 / Step 1: Request Appointment with payment

Select a date and time for your appointment using the calendar below. / Step 2 (below) must be completed with payment to hold your appointment. Part of this process includes creating your Patient Portal account. The instructions for setting up the Patient Portal can be found here.

Please contact your airline to confirm the date and time you need to provide your test result to them. Then, schedule your test appointment accordingly.
Flights with multiple stops or layovers may require you provide test results to them before the first flight departs.
温馨提示：周五早上10点以后到周日取样，IgM结果出在下周一的可能性很高。PCR周六中午10点之后和周日取样，结果出在周一的可能性很高。
爱家诊所强烈建议大家周一到周五上午10点前取样。
航空公司的要求可能会和使馆不同，请大家一定和航空公司确认好，再安排行程和取样时间！预约前请和航空公司确认航空公司的要求。航空公司可能会和使馆有不同的要求！不同航空公司也可能有不同的要求！

  3. Delta flight from Seattle to Beijing
Delta may require your test results before the first flight from DC to Detroit. Please confirm with your airline before scheduling your appointment to ensure enough time to send the test results.

Your appointment will not be confirmed until you complete the form and make your payment in Step 2.

Coronavirus (COVID-19) Test Application for Travel （出行用新冠检测）

Cancellation Policy: You may cancel your appointment for a 50% refund. If the situation is out of our control, such as airline cancels your flight, lab issue which cause delay testing or cancel testing, we can offer NO FEE reschedule, or an 80% refund with proof of airline's cancellation or policy change.

取消预约须知：如果您取消已确定的预约时间，您可以申请退还总费用的50%。如果是因为不可抗拒因素，例如航空公司取消了您的预定航班，实验室原因不能正常检测，政策变化等情况，实验不能如期进行，您可以申请改约或者退还总费用的80%。申请此项退款需持航空公司或使馆官方声明。

Saturday: This is the highest risk for IgM test due to lab processing hours. You may not get your result until Monday afternoon.
Sunday / 周日: We can collect samples at our location on Sundays, but the earliest you may receive your result is Monday afternoon.

Rescheduling Policy / 改约需知: There is no fee to reschedule. / 改约已确定的预约日期，不收改约费。

Lab Hours (subject to change):
PCR Lab Hours 9:00 AM to 2:00 PM Weekdays (not including holidays)
IgM Lab Hours 7:00 AM to 3:30 PM Weekdays (not including holidays)
